Terms of Agreement and Acknowledgment:

By entering into this agreement, the undersigned customer acknowledges their current enrollment in the TakeCare Insurance plan for the ongoing option year 2023–2024. Through the collaboration between TakeCare and Carlson Gracie Academy, the customer (including dependents) is entitled to the benefit of one year of membership with said establishment at no cost.

It is explicitly recognized that the customer bears the responsibility to promptly inform Carlson Gracie Martial Arts and Fitness Academy administration of any changes in insurance coverage, including de-enrollment.

In the event of de-enrollment, the customer is obligated to initiate the re-registration process and commence monthly tuition payments (with the payment start date falling on the month following de-enrollment from insurance). Failure to provide notice within 30 days will result in the voiding of the customer's registration. This measure is in place to ensure the continuity of their membership with Carlson Gracie.

Monthly tuition payments are due on the first week of every month, and the customer agrees to fulfill this obligation. Late payments may incur additional fees or consequences as outlined in Carlson Gracie's payment policy. Carlson Gracie commits to promptly re-activate the customer's membership upon receiving notice of re-enrollment and process monthly tuition payments in accordance with the established billing schedule.

Both parties mutually agree to adhere to the terms outlined in this agreement, and any modifications require written consent from both the customer and Carlson Gracie. By providing acknowledgment, the customer affirms their understanding and acceptance of the terms of this agreement.
